How do I submit the relevant financial relationships form and complete my bio? [Faculty member instructions]

This article is intended for faculty members and presenters.

Scenario: 

All faculty members need to submit their relevant financial relationships in accordance with ACCME Standards for Integrity and Independence. We also encourage faculty members to complete their bios in order to provide learners information about your background and research interests. 

Before getting started, make sure you are logged into the Weitzman Education Platform

Step-by-Step Instructions:

When a Program Specialist marks you as a faculty member for a series or session, you will see notifications at the top of your screen the next time you sign in. 

Select create or review your disclosure on file.

Complete the disclosure form. If you do not have any disclosures, then select No, I have not had a financial relationship with an ineligible company within the past 24 months.

If you do have a disclosure, then select Yes and enter the relevant financial relationship. You may add as many rows as necessary.

Information about what is considered an ineligible company is listed at the bottom of the form. 

Select the Save button when complete. 

Select submit your bio

Fill in your credentials and biography. For the credentials field, do not include your name.

We also encourage you to submit a photo. 

Select the Save button when complete. 


In the future, if you need to report additional financial relationships, select the My Account tab in the top navigation bar and then the Financial Relationships tab
